Party Waiver
Every party guest must have a signed waiver on file before participating.
All participants in Fury Performance Academy birthday parties and private events must have a signed waiver on file. Parents or legal guardians may sign on behalf of minor participants.
By signing this waiver, the undersigned acknowledges that participation in Fury Performance Academy's activities — including dodgeball, flag football, soccer, wiffleball, obstacle courses, relay races, and other physical sports — involves inherent risks including but not limited to falls, collisions, strains, sprains, and other injuries.
Participants assume all risks associated with the activities. Fury Performance Academy, its owners, coaches, employees, and agents shall not be held liable for any injury, loss, or damage that may occur during participation, except in cases of gross negligence or willful misconduct.
In the event of injury, the parent/guardian authorizes Fury Performance Academy staff to seek emergency medical care for the participant. All costs associated with medical treatment are the responsibility of the participant's family.
Fury Performance Academy may take photos and video during events for marketing and social media purposes. If you do not consent to the use of your child's image, please notify us in writing before the event.
Waivers can be signed digitally before the event — contact us at info@furyperformanceacademy.com to request the form — or in person at drop-off. We recommend handling this in advance to keep the party flowing smoothly.
